Implant insertion
Insert the humeral nail along the guide wire and confirm that the proximal end of the humeral nail is 5-10 mm below the cortex to avoid any possible collision with the rotating rotator cuff. Note that the humeral nail should be inserted by hand pressure. After reaching the appropriate depth, it may be necessary to turn the humeral nail and targeting device up to 20° to align the screw with the broken bone while avoiding the biceps tendon. In order to avoid injury to the axillary nerve, the humeral nail should not be inserted deeper than 10.0 mm under the cortex. The groove mark on the target plate can be used to confirm the depth of 5-10 mm under the cortex. Insert a 2.8 mm wire tap drill (HR-D105) through the small hole directly above the center hole on the targeting device to confirm the depth of the humeral nail. The depth can be confirmed with fluoroscopy; the C-arm must be completely parallel to the arm in order to obtain accurate implant depth images. Remember to remove the guide wire before drilling.
